Enrollment and Immunizations

We welcome new students throughout the school year and are committed to making the enrollment process as smooth as possible for families joining our school community.

Required Documents

  • Birth certificate (certified copy)
  • Current immunization records
  • Proof of residency (utility bill, lease agreement, or property tax statement)
  • Previous school records and transcripts
  • IEP or 504 Plan (if applicable)
  • Custody documentation (if applicable)

Immunization Requirements

All students must meet the Arizona Department of Health Services immunization requirements before attending school. Required immunizations include the following:

  • DTaP/DTP/DT (Diphtheria, Tetanus, Pertussis)
  • Polio (IPV/OPV)
  • MMR (Measles, Mumps, Rubella)
  • Hepatitis B
  • Varicella (Chickenpox)
  • Additional requirements for students entering 7th grade

Enrollment Process

  1. Complete the enrollment packet.
  2. Submit the required documentation.
  3. Schedule an appointment by calling Stacy Smith, our district registrar, at 928-204-6748, or Maria Ortega, our district translator, at 928-204-6622.
